I acknowledge that I am the parent or legal guardian of a child participating in a class at Paper City Fabrics. I acknowledge this class and/or workshop is in an environment where machinery will be operated by my child, including but not limited to sewing machines, steam irons, sharp utensils such as scissors and pins and other crafting tools. Use of such equipment carries with it the potential serious injury.

I hereby assume all of the risks of my child participating in this class and/or workshop. I certify that my child is physically able to operate required machinery and equipment without constant supervision. I acknowledge that this Accident Waiver and Release of Liability (AWRL) form will be used by Paper City Fabrics, LLC, and affiliates for any class, workshop or event in which I may participate, and it will govern my actions and responsibilities at said event. 

In consideration of my registration and/or purchase of a class and/or workshop ticket permitting me to participate in this event, I hereby take action for myself, my child, my executors, administrators, heirs, next of kin, successors, and assigns as follows: (A) Waive, release, and discharge from any and all liability of the following entities or persons: Paper City Fabrics, LLC, and their directors, officers, employees, volunteers, representatives and agents, the event sponsors and event volunteers, (B) indemnify and hold harmless all entities or persons mentioned in this paragraph from any and all liabilities or claims made by other individuals or entities as a result of my child's actions during this event. 

I hereby consent my child to receive medical treatment that may be deemed advisable in the event of injury, accident, and/or illness during this event. I understand that at this event or related activities my child may be photographed. I agree to allow my child’s photo or video to be published on Paper City Fabrics website, social media channels, and/or local news media, unless I email Paper City Fabrics to decline. To decline, send an email to info@papercityfabrics.com with the subject line “Decline Photography” before the start of the first class of the session. 

This AWRL shall be construed broadly to provide a release and waiver to the maximum extent permissible under the applicable law.
